I have a dentist who would like to insure his hands. Anyone have access to a market for this risk?

Yes, Lloyd's will insure almost anything. You can contact Peterson International at 1-800-345-8816 to see what they can do for you. :P :P

Most high-quality disability income companies will have options for the dentist as well. Does he want a "lump-sum" one time payout or simply a continued income should he be unable to practice?

If he needs the income, why settle for just his hands. If his brain fails, if he looses his sight or contracts HIV he is still just as disabled as a dentist without the use of his hands...

If he asked for fire insurance on his home because he was worried about a kitchen fire... would you only insure the kitchen?

Do the job right.

You S.L. brokers touting Lloyds need to know that may states have the agent and client sign statements that they (the agent) tried hard to find a domestic carrier before offering a non-admitted (therefore not fully controlled by the state insurance dept.) product.

The syndicates at Lloyd's offer excellent opportunities for placing special risks. That's what they do, and do it well.

However, be careful to do the due diligence and seek proposals from admitted carriers first! Just a word to the wise.
