Which form to write D&O coverage on- opinion needed

If you had a privately held company (family owned) that owned and operated several nursing homes in a 5 states would you write the D&O on a Healthcare policy form or on a Private Entity policy form? What factors do you consider the most important in determining how to classify the risk exposure for the purposes of placing the D&O in a particular market- e.g. privately company or industry (healthcare)?
