Canadian Auto ID Cards
I know there is a lot of debate as to whether or not you need to a Canadian Auto ID card when visiting Canada. That's not the question here. I am simply looking for a company that provides the blank forms so that we can issue the occasional Canadian Auto ID card to our clients whe they request it. Does anyone know where to get the yellow forms? Preferably on 8.5x11 perforated paper?

Our lead insurance company supplies them to us.
